We have recently introduced this style of appointments for our patients with Type 2 Diabetes. NHS England are encouraging Practices to look at other ways of delivering care due to increasing demands on primary care services.
Patients have their annual review appointment in a small group of up to 6 people with the same condition. The atmosphere is relaxed and informal. Patients are seen at the end of the session on a 1 to 1 basis to discuss any personal matters and to complete their review.
This is to promote peer support and enable people to learn from each other how to self-manage their condition.
The session lasts for up to 2 hours in total and allows people to have up to date ongoing education with the Clinician that they normally see. Questions are encouraged and are group led.
The aim is for our patients to have improved health and wellbeing and improved standards of care.
We hope to be rolling this out to other chronic conditions very soon.
